📄 General Information
The Certificate for Return to the Republic of Uzbekistan is issued to citizens of Uzbekistan and stateless persons who are temporarily residing abroad and who either maintain permanent registration in Uzbekistan or were deregistered by court decision.
📍 Application Procedure
The Consulate General of the Republic of Uzbekistan in Washington State provides consular services to residents of the following U.S. states:
- Washington
- Oregon
- Idaho
- Montana
📌 IMPORTANT: Applicants (or parents of minors) must provide proof of residence, such as a lease agreement, driver’s license, or another supporting document.
📑 Required Documents for the Certificate
Applicants must submit the following documents. Documents may be submitted by mail.
1. Application Form
- 3 printed and signed copies of the application form;
- Complete the application online at:
http://consulate.mfa.uz/anketa-new/index - A JPG photograph (up to 90 KB) must be uploaded.
📌 For minors, the child’s information and parents’ details must be included.
2. Photographs
- 2 color photographs;
- Size: 50×50 mm (2×2 inches).
3. Passport
- Original expired passport;
- One photocopy of the passport;
- Main information page;
- Registration (propiska) page;
- OVIR page.
4. Birth Documents
For persons born in Uzbekistan who never received a passport:
- Copy of birth certificate;
- Copies of parents’ passports.
5. Minors (Under 18)
- Birth certificate;
- Passport (if available);
- Copies of parents’ passports.
6. Children Born in the U.S. or Canada
- Birth certificate (original and copy);
- Apostille;
- Copy of Green Card or foreign passport.
7. Proof of Legal Status
- Copy of Green Card; or
- Copy of foreign passport.
This document is mandatory.
8. Consular Fees
- Age 16 and older: $60 USD
- Under age 16: $10 USD

9. Validity of the Certificate
- Issued for 1 month or 3 months;
- Cannot be extended;
- Applications submitted more than 3 months before travel will not be accepted.
10. Passport Requirements
- Expired passports must be submitted;
- Passports are forwarded to the internal affairs authorities in Uzbekistan;
- Processing may take approximately 1.5–2 months.
11. Lost Passport
- Police report (if available);
- Official document confirming passport loss issued by U.S. or Canadian authorities.
12. Important Rules
- A separate certificate is issued for each individual;
- Information about the accompanying adult must be included for children under 16.
13. Cases Where Certificates Are Not Issued
- Foreign citizens;
- Persons who renounced Uzbek citizenship;
- Persons without permanent registration in Uzbekistan;
- Applicants submitting false or incomplete documents.
